Menasha Packaging, ORBIS Corporation, Menasha Corporation Corporate Office: The following benefits package is applicable to Full-Time and Benefit Eligible Part-Time* U.S.-based Salaried and Non-Union Hourly Employees. This is not representative of the benefits package available to non-U.S. based employees or to union employees whose benefits are determined by a union contract. The description below summarizes only the comprehensive benefits options applicable across all locations. Individual locations may have additional benefits that will be shared during the interview process.

While we have made every effort to ensure the accuracy of this information, if there are any differences between this information and an applicable Summary Plan Description, the Summary Plan Description will govern. All applicable Summary Plan Descriptions will be provided upon hire. The Company also reserves the right to unilaterally alter, amend, suspend or terminate benefit options and/or coverage at its sole discretion and at any time.
*A Benefit-Eligible Part-Time Employee is defined as an employee who is regularly scheduled to work more than 20 hours, but less than 40 hours per week in a position expected to last longer than three months.

COMPREHENSIVE BENEFITS PACKAGE DETAIL
Domestic Partner Coverage
Domestic partner coverage is available for medical, dental, vision, supplemental life, supplemental accidental death & dismemberment, employee assistance program, and healthy rewards program/annual health risk assessment. A domestic partner is an established relationship between an employee and one other person of the same or opposite sex that meets certain eligibility criteria and is not formed through a legally sanctioned marriage.
HealthCare Benefits
Medical/Prescription Drug Plans: We are proud to offer three medical plan options which include automatic enrollment in the prescription drug plan. Two of the medical plans are partnered with a Health Reimbursement Account (HRA) and one is partnered with a Health Savings Account (HSA). Each is a PPO plan with in-network and out-of-network coverage and provides different premiums, copayments, deductibles, coinsurance levels, and Company contributions to either your HRA or HSA. Offering multiple plan options allows our employees to determine a plan that best fits their personal situation for a given year.
HRA Plans: The Company contributes funds on your behalf to an HRA which can be used to pay up to 50% of eligible out-of-pocket medical, dental, and vision expenses.
HSA Plan: An HSA is an individually-owned Health Savings Account which allows you to decide how to utilize the dollars for eligible health care expenses. In addition to the Company contributions made to your HSA, you may also make personal contributions to this account.
If you do not elect to participate in a medical plan, you will be eligible for a Waive Incentive that includes both income on your paycheck (subject to applicable taxes) and a contribution to an HRA.
Dental Plan: Our dental plan consists of a three-tier plan design: PPO Provider, Premier Provider, or Non‑Participating Provider. Dental coverage will vary depending on which category your dental provider falls under.
Vision Plan: Our vision plan provides comprehensive vision care with coverage for lenses and frames or contacts, with access to over 7,600 retail outlets or reimbursement for out-of-network benefits.
Spending Accounts
Healthcare Flexible Spending: Utilize pre-tax contributions to pay for eligible out-of-pocket expenses for healthcare costs such as copays, deductibles, coinsurance, and other expenses not covered by insurance. Annual contribution limit is $5,000. Some limitations apply if enrolled in the HSA medical plan.
Dependent-Care Flexible Spending: Utilize pre-tax contributions to pay for expenses incurred to care for dependents to enable you to work. Annual contribution limit is $5,000.
Auto-Reimbursement Program: Auto Reimbursement is a behind-the-scenes electronic progress that automatically transfers participants' eligible out-of-pocket medical and prescription drug expenses from the medical/pharmacy claims processor to the FSA/HRA administrator for reimbursement from your Healthcare Flexible Spending Account and/or Health Reimbursement Account (HRA). This process eliminates the need for participants to submit a manual claim for reimbursement. This process is not available for dental, vision, or HSA medical plan claims.
Disability Benefits
Short-Term Disability: This benefit provides a level of income continuation up to a maximum period of 26 weeks in the event you are unable to work due to a non-occupational illness or injury.
Long-Term Disability: This benefit provides monthly compensation in the event that you become disabled and are no longer able to work. Monthly benefits will commence after completion of a 180-day elimination period.
Survivor Protection Benefits
Basic Life*: The basic term life insurance plan provides financial protection for your beneficiaries by paying a benefit in the event of your death. The Company provides a basic life benefit equal to your annual wage (rounded up to the next $1,000) up to a maximum of $50,000.
Basic Accidental Death & Dismemberment (AD&D)*: The basic AD&D insurance plan provides financial protection for your beneficiaries by paying a benefit in the event of your death from an accident or for you in the event of any other covered loss that results from an accident. The Company provides a basic AD&D benefit equal to your annual wage (rounded up to the next $1,000) up to a maximum of $50,000.
Supplemental Life: You can choose supplemental term life insurance coverage for yourself (up to $500,000), your spouse or domestic partner (up to $250,000), and your dependent children (up to $10,000).
Supplemental AD&D: You can choose supplemental AD&D insurance coverage for yourself (up to $500,000), your spouse or domestic partner (up to $250,000), and your dependent children (up to $100,000).
Business Travel Accident: Business Travel Accident provides accidental death and dismemberment coverage while you are traveling on company business.

Paid Time-Off Benefits
Vacation: We have established a vacation policy to provide employees with a period of rest and relaxation each year without the loss of base pay. The amount of vacation time you are entitled to is determined by your years of service and whether you are a full–time or part–time employee.
Holidays: We have established a holiday policy to provide employees with time off to observe traditional holidays without the loss of pay. Each location has a set schedule of paid holidays based upon the needs of the business.
Retirement Benefits
401(k) Retirement Savings Plan: The 401(k) Plan is a savings and investment plan that allows you to defer some of your pre-tax income for retirement. You are eligible to begin contributing between 1 to 75% of your pre-tax income upon hire. If you are a full-time employee, the Company Match will start after you have completed six months of service. Part-time employees must complete six months of service and work at least 1,000 hours in their first anniversary year or any calendar year thereafter before becoming eligible for the Company Match.
Education Benefits
Continuing Education Program: Education reimbursement is available to benefit-eligible employees who are regularly scheduled to work 30 hours or more per week. It provides for reimbursement of 100% of your tuition, registration, and laboratory fees upon completion of a course with a grade of "C" or better or pass if only pass/fail grade is offered. The maximum financial assistance is $5,250 per calendar year. Employees are eligible when in active status, maintain satisfactory performance, have worked for the Company at least three months before the start of the course for which assistance is being requested, and are employed by the Company on the date the course of education is successfully completed.
Personal Development Program: This program covers training such as certification and licensing preparation and continuing education requirements, seminars, workshops, and conferences.
Other Benefits
Employee Assistance Program: We believe that our employees are our most valuable resource and recognize that there are many personal issues that can affect an employee, such as relationship issues, financial concerns, parenting questions, and so on. Through the Employee Assistance Program (EAP), you and your dependents are eligible for confidential counseling on a wide variety of personal issues from the first day of your employment. The Company pays the entire cost of this program.
Employee Health Advocacy Help Line: Our Employee Health Advocacy Help Line is staffed by an M.D. and R.N. that will work as your advocate to help if you have medical questions, are looking for a doctor, need assistance with claims resolution, received a diagnosis that you want more information about, or have questions about your insurance plans.
Wellness Reimbursement Program: Your health and fitness are important to the success of the Company. We will reimburse benefit-eligible employees for one-half of the cost of wellness expenses, up to a maximum of $125 per year. Covered expenses include membership fees to a commercial health club, weight-loss program fees, healthcare provider-sponsored nutritional seminars, smoking cessation classes, fitness classes, and registration fees for community events such as marathons, walkathons, etc.
Healthy Rewards Program/Annual Health Risk Assessment: Annual routine wellness exams are essential to prevention and early diagnosis of many severe health conditions. In an effort to promote wellness, if you elect one of our medical plans, you and your spouse (if applicable) will be eligible to participate in the annual on-site Health Risk Assessment. Just for participating, you and your spouse (if applicable) will then be eligible for the Healthy Rewards benefit which is a one-time deposit of $150 into your HRA or HSA for your participation and another $150 for your spouse’s participation (if applicable).
Menasha Corporation Foundation
Dependent College Scholarship Program: The Menasha Corporation Foundation awards one- and four-year college scholarships to eligible dependents of employees. Scholarship recipients must be pursuing a bachelor’s degree at an accredited school.
Matching Gifts Program: The Matching Gifts Program is one way you and the Menasha Corporation Foundation can support education. The Foundation will match your personal gifts (dollar-for-dollar, up to a maximum of $1,000 per year per employee) to any accredited school, including K-12 and post-high school institutions such as universities, colleges, or technical schools.
Dollars for Doers: The Dollars for Doers Program provides grants to schools (K-12) to supplement employees’ volunteer work. After an employee has completed 20 hours of volunteer work at a school, the Menasha Corporation Foundation will provide a $250 contribution to that school. In addition, the school will be entered into a $2,500 annual drawing.
